I once had a guy pull the trigger of a long flintlock with his toe to commit suicide. You have to really be committed to dying if you do something like that. I would see where in the scenario the shotgun was located in relation to the body and what part of the body the guy was shot. Positioning the gun differently would make the trigger more reachable. Guns just don't "go off" by themselves. It takes a series of mechanical events to make them fire. If there is a bad sear and the gun fired when the action was closed, its not likely the business end would be pointed at your body (other than your foot, perhaps) when you closed the gun. Additionally, who cleans a loaded gun?
