They are legal for archery hunting but there are requirements for when they can be put up and then left over night on most public land. There are also requirements that they be tagged with the owners name and/or identifying information on most public lands. During some period of he hunting season you are also required to display orange with 15 feet while the blind is occupied.

The regulation book for hunting is a good place to start. It is available on the internet at PGC.com if you choose to look at it.

When orange is required, would a blaze orange T-shirt pinned to the very top of the blind suffice?

I have found that orange marking paint works well and lasts well. I don't want to give you bad advise not sure of doing this on public ground as painting on a tree would be legal. I started using this paint on my own property 3 years ago and still there. I had an old tower stand that I removed the stand part with my bobcat and placed on the ground a couple years prior to that. Until reading the posts on here I was wrong with what I was doing just never gave it thought, but then fixed myself. Its the only ground blind I have the rest are elevated not requiring it.

Yes as long as it is over 100 square inches and visible 360 degrees. I frequently use an orange vest strapped over the top of the blind.
